Submissions to this webform trigger an anonymous email to the Associate Vice President, Finance and Administration in the University of Arizona’s Division of Agriculture, Life and Veterinary Sciences and Cooperative Extension (ALVSCE). Reports of illegal or unethical activities must be appropriately investigated. Information provided in the emails may be used to investigate reports in compliance with applicable ABOR and UA policies, procedures, and practices.

 
Paraphrased from July 26, 2012 memo from President Ann Weaver Hart to the UA Community:
 
The University of Arizona exists to improve people’s lives consistent with the UA mission. It is a moral obligation of the campus community to report acts of crime, harassment, misuse or abuse.  Failure to report such acts perpetuates and encourages them.  If you witness anything you believe to be abusive or criminal, please contact the UA Police Department immediately at (520) 621‑8273, or 911 in case of an emergency.  If you would like to remain anonymous, the UAPD has established a tip line where you can provide information confidentially at (520) 626-0066.
 
If you experience or witness discrimination or harassment, you also should report the matter immediately. Complaints about student misconduct should be directed to the Dean of Students Office.  Complaints related to faculty, staff, vendor or visitor misconduct should be directed to the Office of Institutional Equity.
 
Employees who supervise others, teach students or have management authority over a University-sponsored program or activity must promptly notify the Office of Institutional Equity if they have reason to suspect a violation of the employee Nondiscrimination and Anti-harassment Policy. The University’s Ethics and Compliance Hotline provides the opportunity for reporting suspected ethics or compliance violations 24 hours a day, 7 days a week at (866) 364-1908. The hotline also allows anonymous reports or concerns.
